Hospice and Palliative Care Centers in the Chicago Area
The difference between palliative care and hospice is a common source of uncertainty for many families facing advanced complex illness. Hospice is an important piece of the palliative care continuum.
Both palliative care and hospice take place when a patient is dealing with advanced, complex illness; however, there are several important distinctions. Palliative care is not dependent on prognosis.
Palliative care is for anyone with a serious illness, regardless of life expectancy. It is delivered concurrently with curative, life-prolonging and maintenance regimens regardless of sophistication, in conjunction with existing providers.
Hospice care depends on a prognosis and life expectancy and assists a person to be in comfort with the support of family, emotional and spiritual needs in a medical inpatient hospital setting or home environment designed for end of life issues. They provide pain care management, psychosocial support, and grief support as needed.

Among the resources offered are state-specific advance directives and publications on talking about end-of-life wishes, supporting someone who is grieving, understanding hospice and palliative care services, talking to a doctor about pain or illness and paying for long-term care.

Information is in the form of videos and brochures which are available in Spanish, Chinese, and Russian. This website provides links to additional resources.
