Each year, 1.2 million Americans are diagnosed with diabetes and recently, it has been estimated that 97.6 million Americans aged 18 years and older are prediabetic. Diabetes education can help people manage their diabetes in ways that fit their lifestyle. This webinar will introduce models, approaches and tools for all members of the interdisciplinary rehabilitation team to facilitate quality of life discussions and improve care outcomes. Foundational knowledge regarding diabetes will be interwoven throughout this webinar, particularly for those patients newly diagnosed. Case studies will be utilized to create confidence for members of the interdisciplinary rehabilitation team to support and provide education for people with diabetes.

- Define current recommendations for diabetes self-management education and support (DSMES).
- Describe 2-3 DSMES relevant topic areas for an interdisciplinary rehabilitation team member needed to address diabetes self-management.
